On May 16, the Pharmacy Quality Alliance (PQA) and the Community Pharmacy Foundation (CPF) honored an individual and a community pharmacy, whose innovative practices have improved care quality, medication optimization, and patient outcomes.

Winning in the individual category was Lindsey Valenzuela, PharmD, APh, BCACP, who is Administrator of Population Health and Prescription Management (PHARxM) for Desert Oasis Healthcare. Dr. Valenzuela was recognized for her leadership of a team of 24 pharmacists, three pharmacy residents, and many technicians.

Dr. Lindsey Valenzuela and Team

This team supports thousands of patients, most of whom are living with multiple, complex chronic conditions and may require multiple medication therapies. Valenzuela implemented an innovative, pharmacist-led, personalized care model that provides medication therapy management services and structured care plans to manage disease.

“These awards are a celebration of innovation and progressive practice in community pharmacy. Through the broad uptake of the creative, best practices like those pioneered by Dr. Valenzuela and Towncrest Pharmacy, community pharmacy will ensure its position as a key partner in value-based, patient-centered care.”

“Congratulations to Towncrest Pharmacy also for their transformational work. While I am humbled by this honor, I really share it with all of my team,” said Dr. Valenzuela. “This is only possible because of the hard work my team does every day and the awesome support we get from DOHC senior management. They are truly committed to providing only the best in managed care for our patients.”

As part of this effort, Valenzuela has pioneered the implementation of digital medicines through Proteus Discover, which feature FDA-approved ingestible sensors that help her team improve patient care. She continued, “We are really excited about our ongoing partnership with Proteus Digital Health for helping our patients with digital medicines. It’s already shown great success for a number of our patients.”
