Diabetes Classes

Take advantage of diabetes classes available in English or Spanish. We offer individual, one-on-one classes, as well as group classes where you can connect with others and share your experience.

Is the Program Right for Me?

Your doctor may recommend the Diabetes Management Program if you:

  • Are at risk of long-term diabetes complications, such as kidney failure, blindness, nerve damage, or amputation
  • Had a recent hospitalization due to diabetes
  • Have diabetes that’s not controlled or hard to control
  • Have episodes of low blood sugar levels (hypoglycemia) often

What to Expect

Your program will include:

  • Access to diabetes supplies
  • Lab tests
  • Medication changes to control diabetes and other health problems, such as high blood pressure and high cholesterol
  • Meetings with members of our team to talk about your lifestyle, diet, medications, and goals
  • Prescription refills
  • Referrals to other resources at DOHC

Multidisciplinary Team

Benefit from timely, coordinated care that helps you improve your wellbeing as quickly as possible. Your team will be led by your primary care provider and may also include:

  • Pharmacists and pharmacy technicians
  • Certified diabetes educator (CDE), professional who teaches diabetes self-management and monitors your progress long-term
  • Dietitians
  • Nurse practitioners
  • Registered nurses

What’s a CDE?

Certified Diabetes Educators are health professionals who have special training to help you manage your diabetes. They may be:

  • Registered nurses
  • Registered dietitians
  • Pharmacists
  • Social workers
  • Doctors
